* All standard room has a coffee maker and a small fridge (all paid stuff should have already removed).
* The hotel guests can have food in their own room but not in any of the meeting rooms or public area.
* Please review the Reminders in the registration form and keep coming back to check the Last Minute Update.

Registration Form Related Issues

1. 年齡計算以 12/28/2000 00:00AM 為基準。
   
   
2. Pre-registration 於 12/03 截止。逾期報名者，以 Late Registration 規則辦理。詳細情形請參閱報名表說明。However the system will stay available until 12/07 for you, church representatives, to keep entering data. The online system will be shut down at midnight on 12/08.
   
   
3. 報名時全家填一張報名表即可，以六人為限 (含小孩) 。六人以上請加填另一張報名表。
   
   
4. 18歲以下青少年及兒童無父母或監護人陪同者，須有父母或監護人簽名之緊急醫療授權書方可報名。若有需要，請至下列網址下載緊急醫療授權書。(http://www.immanuel.net/MCCA/WinterConference/download/medical.pdf)
   
   
5. 牧師、全職傳道人及他們配偶的所有費用全免(不含子女、父母) 。請在報名表[事奉]一欄中註明，以便正確計算實際所需費用。
   
   
6. 12/7/2000前，請各負責聯絡同工將貴教會/查經班所有報名表、附件及支票，以 certified mail 寄給Dayton 教會 Jeremy Lu 收，以便註冊組作最後資料處理。
