MCCA 2001 Summer Retreat Registration Instructions

 1.           在報名費及食宿費以外的愛心奉獻，可幫助大會支持許多有困難參加夏令會的弟兄姐妹及慕道友。若您需要這一部份的免稅証明，可在退修會結束前向大會註冊處索取證明。您若在繳交報名費/食宿費上有實際困難，請主動與您的教會或查經班聯絡。

Your love offering, excluding the registration and room &board, will be used to support attendees who have financial difficulties.  This offering is tax deductible.  You may request a tax-deduction statement from the registration prior to check-out.

2.      2.     帶未滿四歲孩童赴會之父母，至少應幫忙幼兒看顧兩堂。大會將贈送錄音帶以補這兩堂之缺席。

Parents with children under age 4 are requested to share the baby-sitting responsibilities for at least two sessions.  MCCA will provide you with cassette tapes for those missing sessions.

3.      3.     大會請專業人員準備豐富的節目給四歲以上的孩童，所以每人必須繳交報名費。

MCCA provide well-organized children programs for age 4 and up.  Therefore, children registration fee is required.

4.      4.     十八歲下無家長陪同者，您須有父母和監護人委託書及醫療授權書，才能報名。

Youth, who are under 18 and are not accompanied by the parents, are required to complete the registration form AND Medical Release form AND Entrustment form with parent/guardians’ signature.

5.       5.    攜帶物品及注意事項:                 

     Things to bring, and Restrictions:

&                聖經、筆記本、筆、盥洗用具、毛巾、肥皂、雨傘/雨衣、枕頭、毯子、床單、枕頭套。

Please bring your own Bible, notebook, pen, bathroom kit (incl. Towel), umbrella/raincoat, pillow, blanket, linen.

&                許多宿舍沒有空調設備，可自備小型電扇。

No air conditioning in most of the dorms, you may bring small electric fan if necessary.

&                會場有空調。怕冷的可帶毛衣或薄夾克。

All meeting rooms are air-conditioned.  Please bring light jacket with you if necessary.

&                請衣著整齊出入聚會場所及餐廳，勿穿拖鞋、睡袍等。

No sandals, pajama etc. allowed at the meeting rooms and cafeteria.

&                請勿攜帶寵物。(校方規定)  No pets allowed on campus. (Campus restriction)

 Fill-out the date of birth for children under 18.

2.          關係欄請填父、母、夫、妻、子、女、兄、弟、姐、妹、友等以便大會安排床位

用餐: 6/29日校方提供晚餐(不包含在膳食費中，自費$5.00)，但請登記以便統計人數通知校方預備伙食:      人

Dinner on 6/29 will be provided, pay at the cafeteria for $5.00, but not included in meal fee.

每個房間只有兩張單人床，孩童(0~11)若要與父母同住，可免繳住宿費，但必須自備睡袋

Two twin-size beds per room.  No lodging fee required for children (0~11) staying in the same room with parents (BYO sleeping bag).  

實際年齡以6/29/2001 12:00AM(午夜)為計算基準。  

Age cut-off date is 6/29/2001 12:00AM (mid-night)  

    優待報名截止日期為6/3/2001支票抬頭請寫MCCA。在6/3之後報名者(包括當場臨時報名)報名費為$35。

Deadline for early registration is 06/03/01.  Please make check payable to MCCA.  Registration fee is $35 after 06/03/01.  

   若如期不能赴會，可申請退費(6/24/01前)，所有食宿費用均全數退回。報名費為大會作業所需基本費用，恕不退還。

Refund (Room/meal) may be requested (before 6/24/01) if you can not attend the retreat (less registration fee).  

報到時請繳交鑰匙保證金，每把$25；床單及毛巾，每套$20；支票抬頭Cedarville University.

Key deposit is $25 per key, linen deposit is $20 per set, please make check payable to Cedarville University when check-in.
